Position Overview:
We are seeking a Family or Adult Nurse Practitioner (NP) to join a multi-specialty practice in the Bronx, NY. This role offers a mix of house calls and office-based care, providing a flexible and dynamic clinical experience in both in-person and telehealth settings.

Work Schedule:
- Monday–Friday
- 3–5 working days per week (flexible arrangement).
- Combination of office, telehealth (1–2 days), and field visits.
Qualifications:
- Master’s in Nursing (MSN) or equivalent.
- Active NYS Nurse Practitioner license.
- Board Certified (Adult or Family NP).
- Open to experienced NPs and new graduates.
- EMR proficiency (MDLand preferred).
- Strong clinical judgment and communication skills.
Key Responsibilities:
- Conduct physical exams and patient assessments.
- Diagnose and manage acute and chronic conditions.
- Order and interpret diagnostic tests.
- Develop and adjust patient care plans.
- Provide patient education and preventive care.
- Perform home visits and in-office consultations.
- Collaborate with physicians and care teams.
- Maintain accurate EMR documentation.
