Position Overview:
Greenlife Healthcare Staffing is currently seeking a Family Nurse Practitioner to fill an opening with a Community Health and Wellness Clinic located in Brooklyn, New York. This role focuses on providing comprehensive primary care to patients of all ages in a collaborative, patient-centered outpatient setting.

Qualifications:
  • Education: Master's degree in Nursing from an accredited program.
  • Licensure: Active New York State Nurse Practitioner license, CME and DEA license.
  • Certification: Must be Board Certified as a Family Nurse Practitioner (FNP).
  • Experience: Strongly preferred 4 to 5 years of experience as a Nurse Practitioner.
  • Language: Bilingual Spanish is a plus.
  • Technical Skills: Proficient in performing physical examinations, ordering and interpreting diagnostic tests, prescribing medications, and utilizing electronic health records.
  • Soft Skills: Excellent communication and interpersonal skills, strong clinical judgment, empathy, patient-centered focus, and the ability to work collaboratively within a multidisciplinary team.

Key Responsibilities:
  • Perform comprehensive physical examinations and patient assessments for individuals and families across the lifespan.
  • Record and analyze patients' medical histories and symptoms.
  • Develop and implement individualized patient care plans, and contribute to existing ones.
  • Order, administer, and analyze diagnostic tests and laboratory results.
  • Diagnose and manage acute and chronic health conditions such as hypertension, diabetes, respiratory infections, and other common illnesses.
  • Prescribe medications and treatments as appropriate, including controlled substances.
  • Educate patients and families on disease prevention, lifestyle modifications, medication adherence, and treatment options.
  • Provide preventive care, including immunizations, health screenings, and routine check-ups.
  • Collaborate with physicians and other healthcare professionals within the family medicine practice to ensure coordinated, whole-person care.
  • Maintain accurate, thorough, and timely documentation in electronic medical records.
  • Provide compassionate, patient-centered care to a diverse patient population.
