Position Overview:

We are seeking a Board-Certified or Board-Eligible Internist to join our team at a nursing home in Brooklyn, NY. In this role, you'll deliver comprehensive care to geriatric and long-term care patients, ensuring optimal health outcomes in a collaborative, mission-driven environment.

Qualifications:

MD/DO degree from an accredited medical school.
Active New York State medical license.
Board Certified or Board Eligible in Internal Medicine.
Prior experience in geriatrics, nursing homes, or long-term care settings preferred.
Proficiency in EHR systems, chronic disease management, and preventive care protocols.
Compassion, strong communication, cultural competency, and teamwork.

Key Responsibilities:

Consult with patients to understand their symptoms and health concerns.
Diagnose and treat acute illnesses (bacterial and viral infections) and chronic conditions such as hypertension, heart disease, and diabetes.
Prescribe or administer medication, therapy, and other specialized care to treat or prevent illness.
Explain procedures and discuss test results and prescribed treatments with patients.
Monitor patients' conditions and progress, and re-evaluate treatments as necessary.
Provide health and wellness advice, including diet, physical activity, hygiene, and disease prevention.
Immunize patients against preventable diseases.
Maintain detailed notes of patient appointments, including tests, treatments, and results.
Refer patients to other medical specialists when necessary.
Provide support and advice to patients receiving long-term care.
Prepare official health documents or records as needed.
Contribute to research on new treatments and methods to prevent or control illness.
