We are seeking a Licensed Clinical Social Worker (LCSW) for a hybrid role at our multi-specialty practice in the Bronx, NY. This position requires a minimum of two years of experience and offers the flexibility of working primarily remotely, with one on-site day per week.

Qualifications:
  • Master of Social Work (MSW) from an accredited program.
  • Active New York State license as a Licensed Clinical Social Worker (LCSW) (required).
  • Active Medicaid and Medicare.
  • Must reside in the tri-state area.
  • Minimum 2 years of clinical experience.
  • Proficiency in psychosocial assessments, diagnosis, evidence-based therapeutic interventions, and electronic health records.
  • Empathetic, culturally competent, excellent communication skills, strong boundaries, and the ability to work independently and as part of a team.

Key Responsibilities:
  • Conduct comprehensive psychosocial assessments and diagnostic evaluations.
  • Provide individual, group, and family therapy using evidence-based modalities.
  • Develop, implement, and monitor treatment plans.
  • Collaborate closely with physicians, nurse practitioners, and other providers within the practice.
  • Maintain timely and accurate clinical documentation.
  • Provide crisis intervention and safety planning as needed.
