Qualifications:
  • Master of Social Work (MSW) from an accredited program.
  • Active New York State license as a Licensed Master Social Worker (LMSW) (required).
  • Must reside in the tri-state area.
  • Minimum 1 year of relevant experience.
  • Fluent in Spanish.
  • Proficient in psychosocial assessments, therapeutic modalities, case management, and electronic health records.
  • Empathetic and culturally competent, with excellent communication, collaboration, and organizational skills, and a commitment to ethical practice.

Key Responsibilities:
  • Conduct psychosocial assessments and provide supportive counseling under supervision, as required for LMSW licensure.
  • Collaborate with the clinical team to develop and implement patient care plans.
  • Provide case management services and connect patients with necessary community resources.
  • Work closely with physicians, nurses, and other providers in a multi-specialty setting.
  • Maintain accurate, timely documentation of patient interactions and services provided.
  • Participate in team meetings and contribute to a coordinated care approach.
