Qualifications:
- Master of Social Work (MSW) from an accredited program.
- Active New York State license as a Licensed Master Social Worker (LMSW) (required).
- Must reside in the tri-state area.
- Minimum 1 year of relevant experience.
- Fluent in Spanish.
- Proficient in psychosocial assessments, therapeutic modalities, case management, and electronic health records.
- Empathetic and culturally competent, with excellent communication, collaboration, and organizational skills, and a commitment to ethical practice.
Key Responsibilities:
- Conduct psychosocial assessments and provide supportive counseling under supervision, as required for LMSW licensure.
- Collaborate with the clinical team to develop and implement patient care plans.
- Provide case management services and connect patients with necessary community resources.
- Work closely with physicians, nurses, and other providers in a multi-specialty setting.
- Maintain accurate, timely documentation of patient interactions and services provided.
- Participate in team meetings and contribute to a coordinated care approach.
