Licensed Master Social Worker (LMSW) – Nursing Home Surveyor / Complaint Investigator – Syracuse, NY

Location: Syracuse, NY (travel position)
Employment Type: Full-time
Pay: $39/hour (premium pay for holidays and overtime)

Position Overview:
Based in Syracuse, NY, the Nursing Home Surveyor / Complaint Investigator assists with surveillance and investigation activities for nursing homes (long-term care), mandated to evaluate facility performance and monitor the quality of care and services provided. As a member of an interdisciplinary survey/investigation team, you'll review allegations of abuse and neglect; perform surveillance activities at nursing homes, Transitional Care Units (TCUs), and Adult Day Health Care Programs (ADHCP) to determine compliance with participation requirements; prepare written reports of investigative findings and Statements of Deficiencies (SOD) within required time frames using Principles of Documentation; participate in emergency surveys at Skilled Nursing Facilities (SNFs); and participate in state monitoring during Immediate Jeopardy situations, among other duties.

Qualifications:
  • Master's degree in Social Work (MSW) from an accredited program.
  • Current New York State Licensed Master Social Worker (LMSW) license.
  • Minimum 2 years of clinical experience in a medical facility, preferably a nursing home (required).
  • 2 years of experience performing nursing home surveillance activities (preferred).
  • Strong interpersonal skills, with the ability to facilitate positive outcomes in adverse or controversial situations.
  • Excellent written and verbal communication, able to relate effectively to providers, physicians, senior medical and administrative personnel, and residents/patients.
  • Superior investigative and analytical skills, with proficiency conducting interviews with facility staff, residents, and families.
  • Ability to work independently with minimal supervision and within a team.
  • Strong computer skills with the ability to learn new programs.
  • Ability to obtain Surveyor Minimum Qualifications Test (SMQT) certification within 12 months of employment (required).
  • Ability to travel to on-site locations.

Additional Requirements:
  • Flexibility with scheduling, as the state determines the work schedule.
  • Availability to work weekends, holidays, and evening shifts on a rotating basis, as needed.
  • This is a travel position.

Key Responsibilities:
  • Participate in surveys or complaint investigations, including allegations of abuse and neglect, and perform surveillance activities at nursing homes to determine compliance with federal and state regulations.
  • Produce written documentation and draft Statements of Deficiencies (SOD) within required time frames, using Principles of Documentation.
  • Assess compliance with the Plan of Correction within the required time frame.
  • Complete data entry in federal and state reporting databases as required.
  • Participate in state monitoring during Immediate Jeopardy situations.
  • Attend meetings as required.
  • Testify in administrative hearings as needed.
  • Adhere to DOH work schedules and to DOH and IPRO policies and procedures.
  • Travel to on-site locations according to the work schedule.
