Position Overview:
We are currently seeking a Licensed Practical Nurse (LPN) to fill openings with a skilled care facility (SNF) located in Queens, New York. This Night shift role is ideal for a dedicated LPN with nursing home experience who is passionate about providing quality care to residents in a long-term care setting.

Qualifications:
  • Education: Graduation from an accredited Practical Nursing program.
  • Licensure: Must have an active New York State Licensed Practical Nurse (LPN) license.
  • Experience: Must have at least one (1) year of experience in a nursing home.
  • Technical Skills: Must be immune to Hepatitis B.
Key Responsibilities:
  • Provide direct nursing care to residents under the supervision of registered nurses and physicians in accordance with facility policies and state regulations.
  • Administer medications and treatments as prescribed, ensuring accuracy and adherence to safety protocols.
  • Monitor and document residents' vital signs, symptoms, and overall condition, reporting any changes to the supervising RN or physician.
  • Assist with activities of daily living (ADLs) and provide compassionate support to residents and their families.
  • Collaborate with the interdisciplinary care team to develop and implement individualized care plans.
  • Maintain accurate and timely documentation of all nursing interventions and observations.
  • Adhere to infection control protocols and maintain a clean, safe environment for residents and staff.
  • Supervise and assist Certified Nursing Assistants (CNAs) as needed to ensure quality resident care.
