Qualifications:
- Education: Completion of an accredited Licensed Practical Nursing (LPN) program.
- Licensure/Certifications:
- Active New York State LPN license.
- Proof of Hepatitis B immunity (required).
- Experience: Must have at least one (1) year of long-term care experience in a nursing home setting or one (1) year of hospital experience.
- Soft Skills: Strong communication, adaptability to rotating shifts, and teamwork.
Key Responsibilities:
- Administer medications and treatments as prescribed.
- Monitor and document patients’ vital signs and health status.
- Collaborate with RNs, physicians, and therapists to implement care plans.
- Assist residents with activities of daily living (ADLs) and mobility.
- Ensure compliance with infection control and facility protocols.
- Respond to emergencies and provide timely interventions.
- Maintain accurate and timely medical records
