We are seeking a Licensed Practical Nurse to join a union position at a mission-driven non-profit organization operating federally qualified health centers. This role focuses on ensuring healthcare access for all through primary care, behavioral health, and health education services.

Qualifications:
  • Education: Certificate from an accredited practical nurse program.
  • Licensure: Current NY State LPN license and registration.
  • Experience: 1-2 years in an ambulatory setting preferred.
  • Technical Skills: Venipuncture, medication administration, immunizations, basic computer proficiency, including typing.
  • Soft Skills: 
    • Strong clinical, communication, interpersonal, and organizational skills; bilingual Spanish preferred.
    • Demonstrated ability to organize care and activities of multiple patients.

JOB DESCRIPTION:
  • Administer medications, treatments, and wound care under RN supervision.
  • Monitor and document patient conditions, vital signs, and progress.
  • Assist with care plan implementation and collaborate with RNs, therapists, and physicians.
  • Educate patients and families on post-treatment care, medications, and recovery goals.
  • Maintain accurate clinical records, including medication logs and shift reports.
  • Ensure compliance with infection control, safety protocols, and facility policies.
  • Provide emotional support and advocate for the patient's needs during rehabilitation.
