Licensed Practical Nurse (LPN) – Hyde Park, NY (#3456)

Position Overview

We are seeking a Licensed Practical Nurse to join our team at a mission-driven non-profit organization operating federally qualified health centers (FQHCs). In this role, you'll help ensure healthcare access for all through primary care, behavioral health, and health education services.

Qualifications:

Certificate from an accredited practical nurse program.
Current NY State LPN license and registration.
1–2 years in an ambulatory setting preferred.
Skilled in venipuncture, medication administration, and immunizations; basic computer proficiency, including typing.
Strong clinical, communication, interpersonal, and organizational skills; able to organize care for multiple patients; bilingual Spanish preferred.

Key Responsibilities:

Administer medications, treatments, and wound care under RN supervision.
Monitor and document patient conditions, vital signs, and progress.
Assist with care plan implementation and collaborate with RNs, therapists, and physicians.
Educate patients and families on post-treatment care, medications, and recovery goals.
Maintain accurate clinical records, including medication logs and shift reports.
Ensure compliance with infection control, safety protocols, and facility policies.
Provide emotional support and advocate for patients' needs during recovery.
