Qualifications:
  • Master of Science in Nursing (MSN) or higher from an accredited Nurse Practitioner program.
  • Active New York State Nurse Practitioner license (required).
  • Board Certified as a Nurse Practitioner.
  • New graduates are welcome to apply.
  • Bilingual in Russian.
  • Proficiency in comprehensive patient assessments, diagnosis, treatment planning, and documentation; comfort with point-of-care testing and portable medical equipment.
  • Highly independent, with excellent clinical judgment, strong communication skills, compassion, and adaptability to different home environments.

Key Responsibilities:
  • Perform comprehensive health assessments and physical examinations in patients' homes.
  • Diagnose and manage acute and chronic health conditions.
  • Develop and implement individualized care plans.
  • Prescribe medications and treatments within the NP scope of practice.
  • Order and interpret diagnostic tests.
  • Provide patient and family education on health maintenance and disease management.
  • Coordinate care with primary care physicians and other healthcare providers.
  • Maintain accurate and timely documentation of all patient encounters.
