Qualifications:
- Education: Bachelor's or Master's degree in Nutrition, Dietetics, or a related field from an accredited institution.
- Licensure: Must have a New York State license as a Certified Dietitian/Nutritionist (CDN) or Registered Dietitian (RD/RDN).
- Experience: Open to new graduates.
- Technical Skills: Proficient in conducting nutritional assessments, developing meal plans, utilizing electronic health records, and providing medical nutrition therapy for various conditions (diabetes, hypertension, obesity, GI disorders, etc.).
- Soft Skills: Strong communication and interpersonal skills, empathy, cultural competence, active listening, patience, and the ability to work collaboratively with physicians, nurse practitioners, and other healthcare providers.
Key Responsibilities:
- Conduct comprehensive nutritional assessments for patients, including dietary history, lifestyle factors, medical history, and anthropometric measurements.
- Develop and implement individualized medical nutrition therapy (MNT) plans based on patient diagnoses, goals, and cultural preferences.
- Provide nutrition counseling and education for chronic conditions such as diabetes, hypertension, heart disease, obesity, gastrointestinal disorders, food allergies, and eating disorders.
- Educate patients and families on healthy eating habits, portion control, label reading, meal planning, and grocery shopping strategies.
- Monitor patient progress through follow-up visits, adjust nutrition plans as needed, and document outcomes in electronic health records.
- Collaborate with physicians, nurse practitioners, and other specialists within the multi-specialty practice to ensure integrated, whole-person care.
- Stay current with evidence-based nutrition research, dietary guidelines, and best practices through continuing education.
- Maintain accurate, thorough, and timely clinical documentation in compliance with regulatory and ethical standards.
- Participate in team meetings, case consultations, and quality improvement initiatives as appropriate.
- Provide group nutrition classes or workshops as needed.
