Position Overview:
The Registered Nurse (RN) provides and coordinates the care of patients, families, and significant others in the ambulatory care setting. This Union position involves assessing, diagnosing, planning, and intervening in actual or potential health problems, including the evaluation of the patient's response to care.

Qualifications:
  • Education: Bachelor's degree in Nursing (BSN).
  • Licensure: Current New York State Registered Professional Nursing license.
  • Experience: Prior experience in ambulatory care, medical-surgical, or outpatient settings is preferred but not required.
  • Technical Skills: Proficient in patient assessment, medication administration, electronic health records (EHR), diagnostic test interpretation, and care coordination.
  • Soft Skills: Excellent communication and organizational skills, critical thinking, empathy, ability to multi-task, and strong telephone communication skills with the ability to use open-ended questions and demonstrate respect.

Key Responsibilities:
  • Assess, diagnose, plan, and intervene in actual or potential health problems for patients in the ambulatory care setting.
  • Coordinate care for patients, families, and significant others, evaluating their response to treatments and interventions.
  • Perform comprehensive patient assessments, including medical histories, vital signs, and physical examinations.
  • Administer medications, treatments, and immunizations as prescribed.
  • Order, interpret, and analyze diagnostic tests and laboratory results.
  • Develop and implement individualized patient care plans in collaboration with physicians and other healthcare professionals.
  • Educate patients and families on disease management, medication adherence, treatment options, and preventive care.
  • Monitor patient progress and modify treatment plans as needed based on clinical findings.
  • Maintain accurate, thorough, and timely documentation in electronic medical records.
  • Participate in quality improvement initiatives and adhere to all regulatory and organizational standards.
  • Provide telephone triage services, using clinical judgment to assess patient needs and direct them to appropriate levels of care.
