RN Case Manager / Utilization Review - Syracuse, NY (#25268)

Location: Acute Care/Academic Medical Center, Syracuse, NY
Employment Type: Full-time
Hourly Rate: $65/hour

Position Overview:

Optimize patient care transitions and resource utilization in a premier academic medical center. Lead utilization review, discharge planning, and interdisciplinary coordination for high-acuity patients while ensuring compliance with EPIC documentation standards.

Qualifications:

Education: ADN/BSN from an accredited program.
Licensure: Current and active New York State Registered Nurse (RN) license is required.
Certification: Basic Life Support (BLS) certification is required; candidates must hold a current certification or be willing to obtain recertification prior to employment.
Experience:
- Must have Pediatric experience.
- Minimum of 1.5 years of recent experience (within the last 2 years) in an acute care hospital setting of similar size and complexity.
- Proven experience in hospital case management within an acute care environment is essential.
Technical Skills: Proficiency in the EPIC Electronic Medical Records (EMR) system is preferred.

Key Responsibilities:

Conduct utilization reviews for appropriate care level/coverage.
Coordinate discharge planning and post-acute care transitions.
Document care coordination in EPIC.
Ensure compliance with payer guidelines and regulations.
Collaborate with ED teams for rapid case management.
Analyze resource utilization trends and recommend improvements.
